• Sign-in via SSM account will be required to ensure only department members are participating.  Ratings and written responses will then be anonymized.

• Individual ratings will not be shared with provider.  All ratings will be aggregated as an average and shared with team members during performance reviews.

• Free text responses may be shared if you agree to share them and will always be shared anonymously.

• When offering feedback, make it constructive -- if negative, please consider how the person might measurably improve their performance.  Positive feedback is always welcome and encouraged.  If you have trouble thinking of feedback, try to offer one thing this person does well and one thing this person could improve.

• Please be honest.  Feedback is a gift that helps us develop individually and as a department.  Thank you for helping us grow.
